The effect of vagotomy on the human gastrooesophageal sphincter.

چکیده
There have been reports of oesophageal reflux following operations performed for peptic ulcer (Clarke, Penry, and Ward, 1965; Windsor, 1964). Reflux has been noted after partial gastrectomy (Windsor, 1964) and also after vagotomy plus a gastric drainage procedure (Clarke et al, 1965). The operation of vagotomy plus gastric drainage could cause reflux oesophagitis to occur either through direct damage to the gastrooesophageal sphincter and phrenooesophageal ligament or indirectly by its effects upon the stomach, eg, by causing gastric retention. In this study, the possibility that vagotomy (plus a drainage procedure) performed for duodenal ulcer might damage the gastrooesophageal sphincter was investigated. The study also yielded additional information concerning the influence of the abdominal vagus nerves on the maintenance and control of gastrooesophageal sphincter pressures in man.

Metoclopramide in gastrooesophageal reflux.
In 30 patients with gastrooesophageal reflux, intravenous metoclopramide (Maxolon) has been shown to increase the gastrooesophageal sphincter pressure. The rise is dose-related until a maximum pressure, proportional to the resting sphincter tone, is reached. The effect is reversed by atropine. Peristaltic waves are increased in pressure by metoclopramide. In 18 patients with gastrooesophageal re...
متن کاملAction of oral metoclopramide on the gastrooesophageal junction in man.
The effect of oral metoclopramide, 10 mg, was studied with intraluminal manometry in 20 patients suffering from symptoms of gastrooesophageal reflux. The drug increased cardiac sphincter pressure and the amplitude of oesophageal peristaltic waves, but gastric motility was stimulated in only half of the patients. The clinical value of these pharmacological effects should be confirmed in therapeu...
متن کاملEffective of highly selective vagotomy upon the lower oesophageal sphincter.
Recently highly selective vagotomy has been suggested as both a cause of gastro-oesophageal reflux and a potential cure. This study was designed to investigate whether this operation produced any change in the resting pressure or the length of the lower oesophageal sphincter in patients undergoing highly selective vagotomy. A group of patients undergoing truncal vagotomy and drainage were also ...
متن کاملEffect of pH changes on the cardiac sphincter.
In 16 normal subjects the pressure characteristics of the cardiac sphincter have been examined. The effect of perfusing the gastric aspect of the sphincter mucosa has been studied by comparing the effects of saline with those of solutions where pH ranged from 1.0 to 8.0. Acid perfusion produced an increase in sphincteric pressure, particularly at pH 3.0. This suggests that a physiological mecha...
متن کاملEffect of esophagus distension on gastric blood flow, gastrin and somatostatin secretion in rat
Abstract Introduction: There are many studies about the inhibitory effect of the esophageal distention (ED) on gastric motility. Recently, it has been shown that ED decreases the gastric secretions. It is well established that the inhibitory effect of ED is mediated by activation of vago-vagal inhibitory reflex. However, there is not any investigation about the effect of the reflex on the gast...
